Potty Training

HI,my daughter is 4 years old and from what we can tell has a lot of signs that point to SPD. She has been potty trained for somewhat of a year.Just peeing though.She will not use the potty to poop.She has to poop in a diaper other than that she wears panties all day.

For the last 6 months or so she has been holding her poop. She will crouch down and try her hardest to not let it out and is very successful. Lately she has spent a lot of time in a diaper in the afternoon just hoping she will poop.and now she is having accidents. I think its because of the diaper. but if she doesn't have a diaper she wont poop.

I'm going crazy not being able to help her with this. I'll give her an enema and immediately she starts screaming and once she has pooped she has such relief. She will poop on her own for a day or so and then we are right back to where we started. Every time I think shes got to be relieved enough to stop holding it in, but she isn't. and communicating to her that she is hurting herself more by not pooping is off the table.

She constantly has a little poop squeezed between her cheeks and stays with a diaper rash. I don't know what to do and am going crazy that now its effecting her peeing as well.

I brought her to the gastro doc to make sure it wasn't anything else going on and she checked out. The doctor put her on a daily miralax dose which seemed to help at first.

Sorry if I haven't explained the situation well ,its a little hard to describe exactly what is going on.

If anyone has a shared experience or advice PLEASE share with me.
