Five year old uncontrollable behavior

My 5 year old grandson throws dirt, spits and yells I will kill you and snot snot snot while he keeps blowing his nose and gathering it to put on someone.
Tells his parents he hates them and hits them. When he visits he runs thru the house yelling and screaming and will not pay attention to rules. He scratches and hits himself and calls himself names when disciplined
